Rationale for the Use of Topical Antimicrobials in the Treatment of Chronic Rhinosinusitis (CRS)

The National Institute of Health (AI49235) and the Mayo Foundation has supported and published research on the immune reaction to fungi in CRS patients. This research was done in collaboration with:

The Division of Allergic Diseases and Department of Internal Medicine;

The Department of Otorhinolaryngology, Mayo Clinic Rochester;

The Department of Laboratory Medicine and Pathology, Mayo Clinic Rochester;

The Departments of Dermatology and Internal Medicine, University of Utah.

The results show CRS is caused by an immune reaction to fungi in which eosinophils rupture when in contact with fungi in the nasal mucous. The eosinophils release major basic protein (MSP) causing pitting in the nasal mucosa. The pitting destroys the cilia and traps the mucous causing bacterial infections and neutrophilic inflammation.

Immunologic testing showed that the helper T-cells of CRS are sensitized by fungal antigen and release both TH1 cytokine, interferon gamma ( INF-gamma) and recruit and activate TH2 cytokines both eosinophils ( IL-5) and B-cells (IL-13). Therefore CRS is a Systemic Hypersensitive Disease.

Mayo showed that topical antifungals reduce mucosal inflammation and reduce inflammatory mediators IL-5 & IL-13, and thus eosinophil migration.
